Your Family’s Emergency Plan

We’ve created a document which we think we’ll make it easier for you to create your family plan.

Family Plan Includes:

• In-State & Out-of-State Contacts (see the bottom for blank city info so you can add your own)
• Evacuation Spots
• Pleasant Grove City, Utah County & State Emergency Phone Numbers.

There are 4 different ways to complete your plans.

1. Pick up a packet and fill it in by hand and glue pictures and make copies.
2. Download & just Print the files, then fill them out by hand, glue pictures and make copies. You can also email the files to your friends or family. Click the appropriate link(s) below.

3. Download the PDF form, fill in the info, print it out, glue pictures and make copies. (The PDF file does not allow you to drag and drop pictures into it, that is why you’ll need to print it before you can add any pictures.)

4. Download the .JPG files and add your text and images using your favorite image editor. (Be sure it will allow you to add images & text. If you are not familiar with how to do this we suggest method 1, 2 or 3.) Click the images below to view larger images or to save them onto your computer.

These are just the first steps you need to take to get started.

If you have any trouble please let us know. We have tried to make it easy for everyone and self explanatory, but we understand it’s a lot of information to take in.

Here is the updated copy of the plan without our city’s info on it. You can add your own info on it to fit your needs. As always this is for personal use and we encourage you to have a family plan.

BLANK No Specific City Info included: We continue to get requests for our original document, and we’re excited that you  want to edit the document to use for your wards or families. So, we have come up with this BLANK PDF file for you to use and edit as you wish. You can use your favorite image editing program to edit the JPG files provided above or use Adobe Acrobat Professional to insert more pages or remove some if needed into or out of, the PDF file. Please keep in mind, that because we do provide you with free PDF downloads of all of the files we create, we do not send out original files. Thanks for your understanding!
